Secure File Upload Portal
Use the below option if your letter instructed you to download the fillable audit forms (found at the bottom of this page) and upload them with the records requested to the Secure File Upload Portal.
Audit Portal
Use the below option if your letter instructed you to login to the Audit Portal. Your login ID and temporary password are included in the letter.
